Print and fill out the following files: SCR Registration Form 2008 (Word document) SCR Medical Release 2008 (Word document) SCR Rules & Policies 2008 (Word document) * If you are having trouble opening the word document from within Internet Explorer (for example, if your AntiVirus software is configured to alert on all scripts, click here. Mail in the following items: 1. SCR Registration Form 2008 (completed) 2. SCR Medical Release 2008 (completed) 3. SCR Rules & Policies 2008 (read completely & signed by camper & parents) 4. Copy of current medical card (front and back) * We do require that you carry your own medical insurance. A copy of your medical insurance card (both sides) with proper contact information should be sent along with your signed medical release form. 5. Deposit of $100.00 made out to "Crystal Wylie" * The $100.00 deposit holds a place in camp for you. We will finalize the camp numbers and collect paperwork and the remainder due during the month prior to your camp session.
